Study of the variability of scapular inclination and the glenoid version - considerations for preoperative planning: clinical-radiological study

چکیده
BACKGROUND Preoperative planning with the aid of imaging methods is a principal factor in successful surgery on the shoulder. This work aims to evaluate the variability of glenoid version, spiralling twist and scapular inclination in relation to the frontal axis. Studies focusing on measuring the variability of scapular inclination in the standardised rest position are lacking in the literature. METHODS We evaluated 104 CT scans of the shoulder. We measured the glenoid version with respect to the scapular axis at three levels. We measured the scapular inclination angle in relation to the sagittal plane and we determined scapular inclination in relation to the frontal axis. Statistical evaluation was performed using the marginal linear model and parameters were estimated using the generalised least squares method, which enables the dependency of measurements performed on the same subject to be taken into consideration. RESULTS The highest values of retroversion are attained by the glenoid in the cranial section (average -9.96°, range -29.7 to +13.2°). Proof of the spiralling twist is the decline in retroversion at the centre of the glenoid (average -2.09°, range -16.7 to +11.6°). Retroversion decreases further in the inferior direction (average -0.5°, range -20.9 to +17.5°). The average thoracoscapular angle is 45.46°, ranging from 13.1 to 69.0°. The average scapular inclination in relation to the frontal plane is 44.54°, ranging from 21.0 to76.9°. CONCLUSIONS During preoperative planning, the surgeon should take into consideration not only the glenoid version in relation to the scapular axis, but also the value of the scapular inclination so as to eliminate possible surgical errors, optimise prosthesis implantation and thus decrease the risk of functional restrictions of the joint. Glenoid morphology in light of anatomical and reverse total shoulder arthroplasty: a dissection- and 3D-CT-based study in male and female body donors
BACKGROUND Placement of the glenoid baseplate is of paramount importance for the outcome of anatomical and reverse total shoulder arthroplasty. However, the database around glenoid size is poor, particularly regarding small scapulae, for example, in women and smaller individuals, and is derived from different methodological approaches. In this multimodality cadaver study, we systematically exam...
متن کاملComparison of Asymmetric Reaming versus a Posteriorly Augmented Component for Posterior Glenoid Wear and Retroversion: A Radiographic Study
Background: Managing posterior glenoid wear and retroversion remains a challenge in shoulder arthroplasty.Correcting glenoid version through asymmetric reaming (AR) with placement of a standard glenoid component and theuse of posteriorly augmented glenoid (PAG) components are two methods used to address this problem. Our objectiveis to report the radiographic outcomes of patie...
متن کاملAccuracy of the Subchondral Smile and Surface Referencing Techniques in Reverse Shoulder Arthroplasty.
Inferior glenoid baseplate tilt relative to the coronal axis of the scapular body has been associated with improved results and fewer postoperative complications in reverse shoulder arthroplasty. However, the native glenoid surface is not always a reliable reference for the true scapular axis. Digital preoperative planning software and advanced imaging now allow surgeons to more precisely deter...
متن کاملAssessment of glenoid inclination on routine clinical radiographs and computed tomography examinations of the shoulder.
BACKGROUND Accurate assessment of glenoid inclination is of interest for a variety of conditions and procedures. The purpose of this study was to develop an accurate and reproducible measurement for glenoid inclination on standardized anterior-posterior (AP) radiographs and on computed tomography (CT) images. MATERIALS AND METHODS Three consistently identifiable angles were defined: Angle α b...
متن کاملTwo-dimensional glenoid version measurements vary with coronal and sagittal scapular rotation.
BACKGROUND Accurate analysis of osseous glenoid morphology is important in treating glenohumeral arthritis and instability. Two-dimensional computed tomography scans are used to evaluate glenoid alignment. Accuracy of this method is dependent on the angle of axial reconstruction in relation to the position of the scapula. The purpose of this study was to investigate the effect of scapular rotat...
